Enrollment
Can I get insurance for my spouse and family, too?
Yes. Family insurance covers you, your spouse/domestic partner and/or dependent children. If you elect this coverage, a benefit in the amount of 50 percent of your coverage will be payable in the event of your spouse's or domestic partner's covered loss. A benefit in the amount of 25 percent of your coverage will be payable in the event of your dependent child's covered loss.
Do I have to take a medical exam or answer a lot of health questions?
No. Medical exams, blood tests or answering health questions are not required to get and or keep this coverage.
Coverage
Do I have to use the cash benefit to pay medical bills?
No. You can use your payment for medical, family or personal expenses of any kind – or spend it in any way you want, for anything you need.
When does coverage begin?
Your effective date will be shown on your certificate/policy of insurance that you will receive within 15 days of our receipt of your activation form. Please note that your effective date may be delayed depending on the billing cycle of your financial institution. Please be sure to review your certificate/policy of insurance upon receipt to determine your effective date.
Can I cancel?/Can I get my money back?
You have a 30-day Free Review. If you are not happy with this insurance for any reason, and you have not submitted a claim that has been paid (or will be paid), just let us know within 30 days of enrollment for a complete refund of your premium and the coverage will be cancelled back to its effective date. You may cancel at any time after that and all billing will stop.
Is there an age cutoff?
There is no age cutoff, however, benefits will be reduced by 50% at age 65.
What if I have other insurance?
This insurance will pay all cash benefits due to a covered sickness or injury regardless of any other insurance you may have.

Federal Insurance Company, a Chubb company, is the underwriter.
What isn't covered?
The following exclusions apply to all benefits or Hazards under this policy. Additional exclusions, limitations or conditions may also apply to specific benefits or Hazards. This insurance does not apply to 1) Cosmetic surgery or care or treatment solely for cosmetic purposes, or complications therefrom. This exclusion does not apply to cosmetic surgery resulting from an Accident if initial treatment of the Covered Person is begun within twelve (12) months of the date of the Accident or to treat congenital defects in covered newborns. 2) any Accident or Sickness caused by or resulting from, directly or indirectly, the Covered Person's commission or attempted commission of a felony or being engaged in an illegal occupation. 3) any Accident or Sickness caused by or resulting from, directly or indirectly any occurrence while the Covered Person is incarcerated. 4) sex changes or the reversal of tubal ligation and vasectomies, artificial insemination, in vitro fertilization, and test tube fertilization, including any related testing, medications, or Physician's services, unless required by law. 5) any Accident caused by or resulting from, directly or indirectly, the Covered Person being intoxicated, while operating a motorized vehicle at the time of an Accident. Intoxication is defined by the laws of the jurisdiction where such Accident occurs. 6) Alcoholism or drug or substance abuse. In addition, the insurance does not apply to any confinement in a detoxification facility or drug or alcohol rehabilitation facility that is not also a Hospital or part of a Hospital. 7) any Accident or Sickness caused by or resulting from, directly or indirectly, the Covered Person being under the influence of any narcotic or other controlled substance at the time of the loss. This exclusion does not apply if any narcotic or other controlled substance is taken and used as prescribed by a Physician. 8) pregnancy, except a Complication of Pregnancy; 9) pregnancy of a Dependent Child, unless required by law. 10) any rest care or custodial care or treatment for any Accident or Sickness. 11) any Accident caused by or resulting from, directly or indirectly, the Covered Person being engaged in or participating in a motorized vehicular race or speed contest. 12) any Accident or Sickness caused by or resulting from, directly or indirectly, the Covered Person participating in military action while in active military service with the armed forces of any country or established international authority. 13) no benefits are payable related to the Covered Person's suicide, attempted suicide or intentionally self-inflicted injury. 14) voluntary abortion, except with respect to You or Your covered Spouse or Domestic Partner where such person's life would be endangered if the fetus were carried to term. 15) any Accident or Sickness caused by or resulting from, directly or indirectly, war, undeclared war, civil war, participation in an insurrection, rebellion, revolution, warlike acts by a military force or personnel, any action taken in hindering or defending against any of these or any consequences of any of these acts regardless of any other direct or indirect cause or event, whether covered or not, contributing in any sequence to the loss. 16) Accident or Sickness arising out of and in the course of any occupation for compensation, wage or profit or which are payable under Occupational Disease Law, Workers Compensation or similar law, whether or not application for such benefits has been made. This policy does not apply to the extent that trade or economic sanctions or other laws or regulations prohibit Us from providing insurance.

How much does this coverage cost?
The cost is $26.25 per month for you or $39.75 per month for you and your family.
How will I be billed for this coverage?
The coverage you select will be conveniently billed to the account provided at the time of enrollment.
Claims
How do I file a claim?
You can receive and complete our Insurance Claim Forms by calling our Claims Department at 1-800-252-2148. A representative would be glad to assist you in filing a claim.
When do I get my cash benefit?
Benefits will be paid in one lump sum after the Covered Person's discharge from the Hospital.
Why do you need my hospital records to process a claim?
These records are required by the insurance company to verify you were hospital confined and that the primary cause of the hospital confinement was due to a covered sickness or injury.
